1. Introduction
The Green Family Training Center (GFTC) is a premier martial arts and fitness gym specializing in Taekwondo and Jiu Jitsu. Our mission is to promote physical fitness, self-discipline, and personal development through structured training programs. This policy outlines the procedures and guidelines to ensure a safe, effective, and enjoyable training environment for all members and staff.
2. General Guidelines
1. Code of Conduct:
- Respect all instructors, staff, and fellow members.
- Maintain a positive and supportive attitude.
- Follow the gym’s rules and regulations at all times.
- Report any misconduct or safety concerns to the management immediately.

2. Safety Procedures:
- Warm-up before starting any training session.
- Use protective gear as required.
- Follow the instructions of the instructors closely.
- Report any injuries or health issues to the instructor immediately.

3. Hygiene and Cleanliness:
- Maintain personal hygiene (e.g., ensure fingernails are appropriate length).
- Wear clean and appropriate training attire and remove jewelry.
- Clean and sanitize equipment after use.
- Dispose of any waste properly.

4. Behavior Management:
- Children: Efforts will first be made to redirect inappropriate behavior using verbal cues. If ineffective, the child may be asked to sit out for one minute or miss a game at the end of class. As a last resort, if a child is too disruptive, we may ask them to sit out of class for the day.
- Adults: A verbal warning will be issued for the first instance of inappropriate behavior. If behavior persists, a written warning will be given. Continued issues will result in subscription cancellation without refund. Depending on the severity of the incident, management may terminate the membership immediately.

5. Membership Cancellation:
- Management reserves the right to cancel any membership at any time for reasons related to safety or behavior, without a refund.
60-Day Cancellation Policy
We value your commitment to our programs and aim to provide the best possible training experience. If you wish to cancel your membership, the following conditions apply:

Notice Period: A written notice of cancellation must be provided at least 60 days prior to the intended cancellation date. Notices can be submitted via email or in person at the front desk.

Payment Requirements: Membership fees will continue to be charged for the 60 days following the receipt of the cancellation notice. During this time, members are welcome to continue attending classes and utilizing services.

Final Billing: After the 60-day notice period, billing will cease, and no further charges will be applied. No refunds or credits will be issued for unused services during the cancellation period.

Special Circumstances: In cases of injury, relocation, or other extenuating circumstances, exceptions may be considered on a case-by-case basis, provided that supporting documentation is submitted.

Prepaid Memberships: For members with prepaid plans, a prorated refund for unused time beyond the 60-day cancellation period will be issued.

Program Transfers: If you wish to transfer your membership to another individual or pause your membership instead of canceling, please contact our front desk to discuss available options.
By adhering to this policy, we can maintain the high quality of our programs while offering flexibility to our members. We appreciate your understanding and cooperation.

6. Class Schedules
- Classes are scheduled Monday to Saturday. Specific times for each level and type of training are posted on the gym’s bulletin board and website.
- Members are encouraged to arrive 10 minutes before the start of their class for check-in and warm-up.

7. Membership and Fees
1. Enrollment:
- New members must complete a registration form and sign a waiver.
- A health and fitness assessment may be required.
2. Fees:
- Membership fees are to be paid monthly or annually.
- Fees for private lessons, seminars, and special events are separate.

- Regular events such as belt promotions, competitions, and seminars are organized.
- Members are required to participate in local, regional, and national competitions based on rank.
- Event schedules and details are communicated through the gym’s bulletin board, website, and social media.

WAIVER & RELEASE OF LIABILITY
By using the services of Green Family Martial Arts and Fitness LLC (“GFMAF”) and requesting GFMAF to instruct, train and/ or coach you through personal training, aerial acrobatics, martial arts training, sparring, weapons training, forms, self-defense, pad holding, grappling, striking, gymnastics techniques, tumbling, weapon techniques and disarms skills (“Activities”), virtually, at your home, at a public location, or at GFMAF’s location, you, as the undersigned, expressly agree and contract, on behalf of your heirs, practitioners, consultants, insurers, executors, administrators, successors and assigns to the following:
1. The Activities, especially the aerial acrobatics, martial arts training, sparring, weapons training, forms, self-defense, pad holding, grappling, striking, gymnastics techniques (tumbling), weapon techniques and disarms skills, offered through the gym GFMAF and any other form of training that may require the use of equipment, which may include, but not limited to mats, pads, sparring equipment, Thai pads, kicking paddles or any other equipment that may be suggested by a GFMAF instructor, are strenuous physical exercises. You recognize that you must be in good physical and mental health to participate in the Activities. You understand that the Activities require physical exertion and represent and warrant that you are physically fit and have no medical condition which would prevent your full participation in the Activities. you understand that it is your responsibility to consult with a physician prior to and regarding your participation in the Activities. If you have consulted a physician, you have taken the physician’s advice. You understand that GFMAF reserves the right in its absolute discretion to refuse your participation in an Activity on medical or fitness grounds.

2. You are in proper physical condition to participate in the Activities, and you are aware that participation could, in some circumstances, result in property damage, abnormal blood pressure, fainting, heartbeat disorders, physical injury and potentially heart attack. You also understand that you could experience muscle, back, or bone injuries during exercise. You understand your physical limitations and are sufficiently self-aware to stop physical activity before you become ill or injured. You are responsible for making sure that you participate in the learning activities from GFMAF within your learning level abilities, skills, mental capacity and ensure that you do not exceed your learning and training capability. You understand that GFMAF instructors or training shall suggest modifications and other physical movements and equipment that are suitable for your level. You are responsible to determine whether such modifications, physical movement, equipment and any training introduced is within your training level, ability, physical or mental capability. You understand that it is your continuing responsibility to inform the GFMAF of any previous medical conditions, injuries or surgeries prior to your first class and any future changes to your medical condition.

3. You specifically acknowledge and agree that your participation in and use of any Activities are entirely at your own risk. You agree that GFMAF shall not be responsible or liable for any misconduct of anyone, including but not limited to its members.

4. You hereby acknowledge that the nature of COVID-19 is such that your action(s) as a student and participant in the Activities is such that is capable of affecting you, other students, participants, and every other person you interact with or every person who may be in the same environment as yourself. You understand that during the Activities, you may come into close contact with other students, and participants. You understand that there is a potential you could be exposed to and contract COVID-19.

5. You understand that your participation in activities is entirely voluntary and at your own risk. Use of a facemask and social distancing is entirely placed on you and/or your legal guardian. You fully understand the scope of the Activities and the potential risks, including those related to COVID-19, involved in your participation of the Activities. You agree to assume the risks of your participation in the Activities, including the risk of catastrophic injury, death, or exposure to and infection of any communicable diseases.

6. In further consideration of being permitted to participate in the Activities, you knowingly, voluntarily and expressly waive any “Claims” (as defined below) you may have against GFMAF, its owners, members, employees, and/or its instructors, teachers, employees, volunteer staff, interns, and/or independent contractors and any landlord or property owner where you participate in the Activities (each, a “Released Party”) for any Claim that you may sustain as a result of participating in the Activities with GFMAF even if the Claim arises from the carelessness or negligence of any Released Party or any other third party. You agree to indemnify and hold harmless each Released Party from any loss or liability incurred in defending any Claim made by a third party, yourself or anyone else making a Claim on your behalf, even if the Claim is alleged to or did result from the carelessness or negligence of any Released Party or anyone else. “Claims” include but are not limited to any and all liabilities, claims, demands, expenses, fees, legal actions, rights of actions for damages, property damage, personal injury, mental suffering and distress, or death that I may suffer, my children may suffer or that my unborn child may suffer (including any legal fees or expenses) in connection with participation in any Activity.

7. You grant GFMAF the absolute, irrevocable right and unrestricted permission to use, preserve in permanent form and electronically transfer my likeness, image, photo and/or appearance as such that may be embodied in any photos, video recordings, or digital images and the like, taken or made on behalf of GFMAF or auxiliary organizations in perpetuity. You agree that GFMAF has complete ownership of such material and can use said material for any purpose consistent with GFMAF’s missions, programs, products, activities, promotions, advertisements, and the like. These uses include, but are not limited to, videos, publications, advertisements, news releases, websites, multimedia production, internet distribution, closed circuit exhibition, illustrations, bulletins, videotapes, reprints and/or copies/reproductions, edits, copyright, licenses, and any promotional or educational or sales materials in any medium now known or later developed. You waive any right to inspect or approve the aforementioned, acknowledge that you will not receive any compensation for the use of such pictures, images, video, likeness, voice, etc. and hereby release and discharge GFMAF, and its agents, representatives, executors, administrators, assignees, next of kin, and/or any other persons acting on your behalf - or on behalf of your estate by reason of this authorization from any and all claims and demands arising out of or in connection with the use of your name, likeness, image, voice and/or appearance, including any and all claims for invasion of privacy, right of publicity, misappropriation or misuse of image, and/or defamation.

9. You, your heirs or your legal representatives forever release, waive, discharge and covenant not to sue any Released Party for any Claim caused by any negligence or other acts of a Released Party.

10. This waiver and release shall be construed in accordance with, and governed by, the laws of the State of California.
